Absentee Ballot Request Form - Election 2006-1

Notice of Election 2006-1

Absentee Ballots cannot be requested by email, fax or phone; you must use the form above. The Election Committee will mail an Absentee Ballot Package to the home address you provide within five (5) business days of receiving your signed statement. If you do not receive the Absentee Ballot Package within 10 days of mailing your signed statement, please contact the Election Committee at 212-432-2120 or election@actuaw.org. Once you receive your Absentee Ballot Package, you'll follow instructions like the "Sample Instructions For Absentee Ballot Voting" below.

If you request an Absentee Ballot, you must use it to vote, and may not vote at the Membership Meeting. Mail your ballot soon enough for it to be received by May 15, 2006 at 5:00pm. Ballots received after this time will not be counted.

Sample Instructions For Absentee Ballot Voting

Read the following instructions carefully before marking and mailing your ballot:
  1. Verify that you have received all of the following:
    • If you are a member of the New School Unit: the BALLOT FOR NEW SCHOOL UNIT
    • If you are a member of the NYU Unit: the BALLOT FOR NYU UNIT
    • An envelope labeled Secret Ballot Envelope
    • An envelope pre-addressed to the UAW Local 7902 Election Committee
If you have not received one or more of these items, contact the Election Committee at 212-432-2120 or election@actuaw.org.
  2. Mark the ballot(s) by drawing an "X" in the box next to your choice in each race (the New School Unit ballot contains only one race; the NYU Unit ballot contains two). Do not place your name, initials, or any other identifying information on the ballot.
Ballots containing any identifiable information, or marks next to both choices in a race, will be voided.
  3. Place your marked ballot(s) in the envelope labeled Secret Ballot Envelope and seal it. You must use the Secret Ballot Envelope to ensure that ballot secrecy is maintained. Do not write on this envelope.
Marks on the Secret Ballot Envelope, or failure to use the Secret Ballot Envelope, will void your ballot(s).
  4. Insert the sealed Secret Ballot Envelope into the envelope pre-addressed to the UAW Local 7902 Election Committee.
  5. Seal the pre-addressed envelope and PRINT your name and address in its upper left corner. Failure to print your name and address on the outer envelope may void your ballot, because your name, and the number printed in the lower left corner of the envelope, will be combined to identify you as an eligible voter. Your vote will remain secret because the Secret Ballot Envelope – containing your marked ballot(s) – will be separated from this outer envelope, and randomly mixed with other Secret Ballot Envelopes before it is opened.
  6. Place adequate postage on the outer envelope and mail it soon enough for it to be received by May 15, 2006 at 5:00pm.
Ballots received after this time will not be counted.
Ballot secrecy can be preserved only if you personally mark and mail your ballot. Do not allow anyone else to mark or handle your ballot. Do not give your ballot to anyone else for mailing.

If a ballot is spoiled, you may obtain a new one by contacting the Election Committee at 212-432-2120 or election@actuaw.org. If we receive more than one ballot from you, only the last ballot received will be counted.

You must use this Absentee Ballot to vote in this election; you may not vote at the Membership Meeting.
